When you find yourself between hospital-level care and returning to your home and work, The Transitional Care Center at Kirkhaven is the perfect place to regain your health and get back on your feet. Our transitional care unit, housed in its own wing of Kirkhaven, features a fully equipped rehabilitation suite with a specialized staff.

At Kirkhaven, we're small enough to render personalized care, and yet we're fully-equipped to offer a variety of specialized services. We have provided rehabilitation services since 1995, and with the help of a generous donor, we now have an entire wing dedicated to both rehab and transitional care.

Kirkhaven is actively expanding the conditions and levels of acuity for which we can provide care. Our Transitional Care Center capabilities help meet the demand for more affordable health options, especially for those who do not need or desire to be in a hospital.

Our goal is to help residents become as independent as possible...as quickly as possible. There's no feeling of "assembly line" care here!

 

Personalized Care Plan
Again and again, our residents tell us they don't have that "large institution" feeling when they're at Kirkhaven. Each resident works with the Kirkhaven rehabilitation team in developing and implementing a Personalized Care Plan with specific, step-by-step goals.

Once a week, the resident's care team meets to review the progress made and make any adjustments to the plan. We're always flexible in meeting a resident's needs. Members of a resident's care team may include:

  • geriatrician
  • nurse
  • certified nursing assistant
  • social worker
  • physical therapist
  • physical therapy assistant
  • occupational therapist
  • certified occupational therapy assistant
  • speech pathologist
  • audiologist
  • recreational therapist
  • dietitian.

Medical Supervision

is On-Site
At Kirkhaven, the care is always "up close and personal." Geriatricians are on-site and maintain admitting privileges to Rochester General Hospital. Furthermore, our staff regularly communicates with the resident's primary physician.

 

Fully-Trained,

Dedicated Staff
We are blessed with an exceptional staff of fully-trained, dedicated people. To stay current with the latest rehabilitation techniques and equipment, their skills and training are updated on a regular basis. Our nursing staff is trained in the use of PICC lines and central lines for residents who need IV therapy.

The Patient's Family is Part of the Team
We encourage family members to participate in a patient's rehabilitation. Their presence often aids the recovery process, and so, they're welcome to visit at any time and take part in care conferences. We provide support to families to help them cope with the lifestyle changes caused by a loved one's need for rehabilitation.
