Discharge Planning
Your discharge plan begins as soon as you're admitted. Your care team meets weekly to review your progress and adjust your plan. Knowing your goal is to return home as quickly as possible, the team works to ensure that you feel comfortable with your ability to take on the daily activities of your life: shopping, preparing meals, housekeeping, negotiating stairs, getting in and out of cars, etc.

Your care team helps arrange your move back home and provides support for any lifestyle changes that may result from increased needs. Our social workers are available to meet and discuss any specific needs or concerns you or your family may have.

At-Home Services We can also arrange home care services for you. Through Seniorsfirst At-Home Services, we offer a broad array of affordable services for temporary or long-term situations.
